Our vision and ambition

Joint Forward Plan

Our Joint Forward Plan (JFP) sets out the plans in place across the local NHS and wider Devon system and the key milestones for delivery over the next five years.

Published in June 2023, it is NHS Devon’s response to the One Devon system’s Integrated Care Strategy. Both of these documents together make up the Devon Plan.

Additionally, there is an immediate requirement to stabilise the financial position and recover activity, to improve operational performance, access and quality of care.

To achieve both of these, we need to transform the way we work together across our system.

The JFP is focused around nine key delivery programmes and 10 enabling programmes.

Delivery programmes

  1. Acute Services Sustainability
  • Deliver high quality, safe, sustainable and affordable services as locally as possible.
  • Stabilise care in the short term through increasing productivity and capacity
  • Sustain care in the medium term making the best use of resources
  • Transform care in the longer term working as one joined up system.
  1. Children and Young People
  • Provide services for children who need urgent treatment and hospital care as close to home as possible
  • Deliver safe and personalised maternity care
  • Proactively address health inequalities
  • Develop family hubs and early help models
  • Prioritise SEND and embed reforms
  1. Community Development and Learning
  • Place local communities at the heart of decision-making
  • Agree collective community goals
  • Support community development workforce
  • Integrate community partnerships into LCP infrastructure
  1. Employment
  • Reduce number of 16-18-year-old and care-experienced people who are NEET
  • Reduce number of individuals with disability/mental health need who are unemployed
  • Support more people including unpaid carers into employment
  1. Health Protection
  • Reduce health care associated infections
  • Improve update of school-age immunisation
  • Improve vaccine coverage, particularly measles, mumps and rubella (MMR) and in priority groups
  • Improve uptake of cervical and breast screening
  1. Housing
  • Improve poor quality housing
  • Improve identification and recognition of need for specialist housing, accommodation for older people and affordable housing
  • Reduce number of people who are homeless
  1. Mental Health, learning Disability and Neurodiversity
  • Improve population mental health and wellbeing
  • Improve outcomes and experiences of people with mental illness
  • Develop sustainable supportive community offer
  • Deliver annual health checks and action plans for people on GP learning disability registers
  • Improve autism diagnostic pathways
  • Reduce reliance on locked and secure inpatient care
  1. Primary and Community Care
  • Deliver an integrated, collaborative model of care
  • Develop a proactive, preventative and personalised approach, supporting people in their own homes
  • Develop sustainable, high quality general practice
  • Ensure a sufficient, sustainable care market
  1. Suicide Prevention
  • Reduce the rate of suicides towards or below the national average
  • Develop and deliver local partnership action plans informed by insight, data and need and aligned to the national and local evidence base and policy
